Kenya received its first consignment of Lenacapavir, a long acting injectable pre-exposure prophylaxis intended to prevent new HIV infections, marking a significant development in the country’s national response. The shipment of 21,000 starter doses arrived through a partnership with the Global Fund and will be stored and distributed by the Kenya Medical Supplies Authority ahead of a phased rollout set to begin in March. The Ministry of Health confirmed that the initial deployment will target 15 high burden counties under the National AIDS and STI Control Programme as part of efforts to expand prevention options beyond daily oral medication.
The intervention marks one of the most consequential adjustments to Kenya’s prevention strategy in more than a decade. With HIV prevalence at 3.7 per cent and approximately 1.34 million people currently on treatment, prevention remains central to easing long term fiscal and clinical pressure on the health system. Officials estimate that 41 per cent of new infections occur among young people, reinforcing the urgency of diversifying prevention tools beyond daily oral PrEP.
Unlike conventional daily regimens, Lenacapavir is administered twice a year through subcutaneous injection. By targeting key stages of the HIV life cycle, it prevents the virus from establishing infection in HIV negative individuals at substantial risk. The shift from daily pills to biannual dosing is expected to significantly improve adherence, particularly among populations affected by stigma, pill fatigue, mobility constraints, and inconsistent access to facilities.
The economic implications are substantial. The government has negotiated a cost of approximately Ksh 7,800 per patient annually, a dramatic reduction from the medicine’s earlier global price point. At this rate, the intervention becomes fiscally defensible within Kenya’s expanding Universal Health Coverage agenda, reducing long term expenditure associated with lifelong antiretroviral therapy. Preventing infections remains far more cost effective than sustaining treatment for decades.
Operationally, the rollout will test the resilience of Kenya’s pharmaceutical and logistics systems. The Kenya Medical Supplies Authority must integrate the injectable into existing cold chain and distribution frameworks while ensuring continuity between starter and continuation doses. An additional 12,000 continuation doses are expected by April, with a further 25,000 pledged by the United States government to reinforce early scale up. Reliable forecasting, stock management, and last mile delivery will determine whether the intervention achieves momentum or faces bottlenecks.
Regulatory groundwork has already been laid. In January 2026, the Pharmacy and Poisons Board completed a scientific review and registered both oral and injectable formulations for use in Kenya, following prior approval by the U.S. Food and Drug Administration in June 2025 and endorsement by the World Health Organization. This layered regulatory process strengthens public confidence while aligning Kenya with international pharmacovigilance standards.
The phased deployment across Mombasa, Kilifi, Machakos, Nairobi, Kajiado, Nakuru, Uasin Gishu, Kakamega, Busia, Siaya, Kisumu, Migori, Homa Bay, Kisii, and Kiambu reflects a data driven targeting model anchored in national HIV burden mapping. Subsequent phases will extend coverage nationwide. County level readiness, healthcare worker training, and patient education campaigns will be critical in ensuring proper initiation protocols and safe administration.
Healthcare workforce capacity will face both opportunity and strain. Nurses and clinicians require updated training to administer the injection, monitor side effects, and counsel clients transitioning from oral PrEP. Clinics must adapt scheduling systems to manage six month return visits while maintaining routine HIV testing protocols. However, reduced daily pill dispensing may ease pharmacy congestion over time.
The initiative underscores the importance of public private and international collaboration. Support from the Global Fund and the United States government reflects Kenya’s continued reliance on blended financing models for high impact interventions. At the same time, domestic policy coordination through NASCOP and the Ministry of Health signals growing institutional maturity in adopting advanced biomedical tools.
If effectively implemented, Lenacapavir could significantly reduce new infections, particularly among adolescents, young women, and other high-risk groups. Fewer infections would translate into long term savings, improved productivity, and reduced pressure on overstretched treatment programs. Prevention success also strengthens Kenya’s standing as a regional leader in HIV response across East and Southern Africa.
Kenya’s decision to move swiftly on Lenacapavir demonstrates more than pharmaceutical adoption. It signals a strategic pivot toward long acting, patient centered prevention, stronger regulatory oversight, and integrated supply chain management. In doing so, the country positions itself not only to curb new infections but to modernize its broader health delivery systems in pursuit of sustained epidemic control and universal health coverage.
